Key Responsibilities\n

As a Licensed Clinical Social Worker, your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments through interviews, observations, and a review of medical records to thoroughly evaluate the emotional, social, and environmental factors affecting each patient's recovery process.

• Providing empathetic counseling and support to patients and their families, addressing their concerns regarding adjustment difficulties, emotional well-being, and coping strategies during rehabilitation.

• Working collaboratively with our rehabilitation team to create individualized treatment plans that carefully consider the unique psychosocial needs of each patient, adjusting strategies as required based on progress.

• Assisting patients in identifying and accessing essential community resources, such as support groups, vocational rehabilitation, and housing assistance, that facilitate successful transitions to independence.

• Educating patients and their families about available community resources and post-rehabilitation services, ensuring they are well-prepared for life after therapy.

• Participating in discharge planning processes and helping to facilitate smooth transitions back to home or other care settings, ensuring continuity of care.

Patient Population / Client Focus\n

In this role, you will support a diverse range of patients, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Your work will play a crucial role in enhancing these individuals' emotional and social well-being while promoting functional independence and community reintegration.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required to ensure a high level of expertise in clinical practice.
