Job Requirements

POSITION SUMMARY

The Clinical Oncology Navigator is a registered nurse serving as a patient advocate, educator, and care coordinator across the cancer continuum in the Health First Integrated Delivery Network (IDN). The Clinical Oncology Navigator facilitates timely access to care, reduces barriers, and ensures a seamless experience for patients that have been diagnosed with cancer. The Clinical Oncology Navigator supports in improving clinical outcomes, patient satisfaction, and operational efficiency.

PRIMARY ACCOUNTABILITIES

1. Conducts care coordination duties that includes being a guide for patients from diagnosis through treatment and survivorship.
2. Collaborates with multidisciplinary teams including licensed practitioners, dietitians, social workers, financial advocates, and infusion Associates.
3. Aligns appointments and services to reduce delays and improve access for patients.
4. Identifies and addresses barriers to care; such as transportation, financial toxicity, and psychosocial distress.
5. Provides emotional support and education tailored to each patient's diagnosis and treatment plan.
6. Monitors treatment compliance and assistance with referrals and authorizations, while participating in tumor board discussions.
7. Documents outreach of encounters and care for coordination activities in the Electronic Medical Record (EMR) system.
8. Educates patients and families on treatment of modalities, side effects, and survivorship planning.
9. Serves as the central point of contact for all Oncology navigated patients.
10. Offers support to patients with decision-making through strong communication, critical thinking skills, and patient engagement.
11. Interprets licensed practitioner progress notes, to ensure all appropriate orders have been placed for follow-up, and treatment plan decisions are carried out.
12. Aids leadership to create and monitor the effectiveness of processes and policies toward improving quality outcomes; actively participates in quality improvement.

Work Experience

MINIMUM QUALIFICATIONS

• Education: Associate's degree in Nursing (ASN) or Nursing Diploma
• Work Experience: One (1) year experience in Oncology Nursing.
• Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement.
• Certification:

1. Chemotherapy and Biotherapy Administration Course certification within six (6) months of start date and maintained.
2. Oncology Nurse Navigator (ONN) certification within one (1) year of start date and maintained.

• Skills/Knowledge/Abilities:

1. Strong communication, critical thinking, and patient engagement skills.
2. Ability to work independently and across multiple locations.

PREFERRED QUALIFICATIONS

• Education: Master's degree in Nursing (MSN).
• Work Experience:

1. Five (5) years' experience in Oncology Nursing.
2. Experienced in infusion and care coordination.

PHYSICAL REQUIREMENTS

• Majority of time involves sitting or standing; occasional walking, bending, and stooping.
• Long periods of computer time or at workstation.
• Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
• May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
• Communicating with others to exchange information.
• Visual acuity and hand-eye coordination to perform tasks.
• Workspace may vary from open to confined.
• May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
