This is an excellent opportunity for an experienced Patient Care Coordinator who can take ownership of patient coordination, communicate effectively, and provide exceptional service to the individuals and families we support.

⭐ MUST-HAVE QUALIFICATIONS

? Previous experience in patient care coordination, healthcare coordination, home care, case management, intake, or a related healthcare role is REQUIRED.

? Fluent in English with strong verbal and written communication skills.

? Spanish-speaking is preferred — candidates who can communicate effectively with Spanish-speaking patients and families are highly encouraged to apply.

? Excellent organizational and follow-up skills.

? Comfortable handling multiple patients, schedules, calls, and priorities.

? Professional, compassionate, and patient-focused.

? What You'll Do

• Coordinate and monitor patient care and services.

• Serve as a primary point of contact for patients, families, caregivers, and referral sources.

• Communicate with patients and families regarding services, schedules, care needs, and follow-ups.

• Communicate with Spanish-speaking patients and families when applicable.

• Coordinate caregiver assignments and patient schedules.

• Follow up with patients and caregivers to ensure services are being provided appropriately.

• Address questions, concerns, and service-related issues professionally and promptly.

• Coordinate with internal staff, caregivers, healthcare professionals, and referral partners.

• Maintain accurate patient records, documentation, and care-related information.

• Monitor changes in patient needs and communicate updates to the appropriate team members.

• Assist with intake, admissions, assessments, and other coordination activities as needed.

• Ensure timely follow-up on outstanding documentation and requirements.

• Maintain confidentiality and follow agency policies and applicable healthcare regulations.

• Provide compassionate, professional customer service throughout the patient’s care journey.
