Job TitleJob DescriptionCoordinate and implement treatment plans by arranging for transition to the next level of medical care in compliance with the multidisciplinary treatment plan.Conducts therapeutic intervention with individuals, families and/or groups to lessen the impact of acute, chronic and/or terminal illness or injury on the lives of patients and families.Provide ongoing risk and case management to evaluate and ensure appropriate levels of service.Organize community services on behalf of beneficiaries; and develop and coordinate procedures for the use of community services by related staffs and satellite facilities.Educate medical staff on the psychosocial impact of injury, outpatient procedures and community treatment resources.
