We are seeking a Part-Time Case Manager to join our team. This position offers 20–25 hours per week (Mon - Fri between 8a - 4:30p) and provides an opportunity to support vulnerable individuals and their families by helping them access services, achieve personal goals, and build meaningful connections within their communities.
As a Case Manager, you will:
- Coordinate services and supports for individuals and families.
- Develop person-centered support plans based on assessed needs, goals, and preferences.
- Monitor and evaluate services to ensure needs are being met.
- Assist individuals in accessing community resources and services.
- Help identify and connect with qualified service providers.
- Educate individuals and their legal representatives about available service options.
- Support self-direction, independence, and community integration.
Qualifications
- The ideal candidate will be self-motivated, organized, and skilled at problem-solving.
- Successful applicants will be able to manage priorities effectively, work independently, and collaborate as part of a team.
- Bachelor's degree from an accredited four-year college or university in Social Work, Psychology, Sociology, or another Human Services-related field.
- One year of experience in working with individuals with developmental disabilities or related conditions
