Key Responsibilities\n

• Conduct thorough assessments to identify the emotional and behavioral issues of patients, developing appropriate crisis intervention strategies as needed.

• Collaborate effectively with physicians and other healthcare professionals to report on patient progress, address specific concerns, and strategize on treatment plans.

• Maintain meticulous clinical and progress notes in alignment with department policies, ensuring accurate and timely record-keeping.

• Engage in the development of individualized treatment and discharge plans based on comprehensive psychosocial assessments and recognized social work interventions.

• Build and nurture relationships with leaders within community resource groups, representing Empower Well Health Care Center at meetings and serving on committees focused on addressing the social needs of our patient population.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling is required.

• Current independent licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) is essential.

• A minimum of two years of professional experience in a clinical setting is mandatory.

Preferred Experience\n

While two years of clinical experience is required, candidates with additional experience in outpatient settings, substance use treatment, or a background in crisis intervention will be given preference. Familiarity with electronic health record systems and case management tools is also advantageous for this role.
