**Exciting Opportunity for a Home Health Physical Therapist!** **Job Description:** - Are you ready to make a real difference in patients' lives? - Join us as a Physical Therapist in the vibrant San Jose area! - Your mission: - Restore function and minimize disability due to disease or injury. - Help clients achieve maximum performance within their capabilities. - Key Responsibilities: - Evaluate and treat patients using medically prescribed physical therapy programs. - Supervise Physical Therapy Assistants and guide them in delivering exceptional care. - Conduct initial admission and evaluation visits. - Notify the Clinical Team Manager of OASIS scores in a timely manner. - Document observations, clinical findings, problems, interventions, treatments, goals, and discharge plans. - Perform additional duties consistent with your skills and training, aligned with our mission. - Territory Coverage: - Explore and serve the beautiful regions of: - Milpitas - Saratoga - Monte Sereno - Los Gatos - Campbell - San Jose - Morgan Hill - San Martin - Gilroy - Coyote Point - Sunnyvale - Requirements: - A minimum of one year of Home Health experience. - BLS/BCLS certification required. - Flexibility in scheduling is essential, with shifts generally from 8am to 5:30pm. - Commitment to working one weekend day per month. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
