Social Worker

Epic Healthcare - NOW HIRIN: Social Worker We’re seeking a compassionate and motivated Social Worker to join our dedicated team. In this role, you’ll play a key part in supporting the emotional and psychosocial well-being of our residents, working closely with families, interdisciplinary team members, and community partners. Social Worker Responsibilities: Assist residents and families in adjusting to short-term rehab or long-term care placement Conduct psychosocial assessments and care planning Coordinate discharge planning and referrals Advocate for residents’ rights and help resolve concerns Participate in care plan meetings and interdisciplinary team discussions Maintain accurate documentation in compliance with state and federal regulations Social Worker Requirements: Bachelor’s or Master’s degree in Social Work (BSW or MSW) Prior experience in long-term care or healthcare setting preferred Strong interpersonal, organizational, and problem-solving skills Knowledge of federal and state regulations related to nursing facilities Empathetic, professional, and resident-centered approach Social Worker Why Join Us?
