Position Overview Seeking a dedicated Licensed Practical Nurse to provide high-quality clinical care to patients in their homes. The LPN will work under the direction of a registered nurse and follow established plans of care while ensuring compliance with all regulatory and agency standards. This role requires strong clinical skills, attention to detail, and excellent communication to support patients, families, and care teams. Schedule & Work Details Schedule: Monday through Friday, 8:00 AM to 4:30 PM Hours: 40 hours per week, 8 hour days Pay Rate: $28 to $30 per hour Coverage Area: 50 mile radius of Troy, Alabama office On Call/Weekend: 1 to 2 weekends per month participation in on-call rotation Mileage Reimbursement: Yes, between patient visits Key Responsibilities Provide high-quality clinical services in accordance with the plan of care and state nursing regulations Adhere to all infection control procedures and safety standards Monitor patient condition and report nursing needs to supervising RN Communicate effectively with physicians, patients, families, and agency staff regarding care updates and concerns Manage visit assignments, scheduling changes, and coordination with schedulers Complete and submit OASIS data sets, coding requests, and required documentation accurately and on time Deliver services as ordered by the physician and compliant with applicable laws and regulations Maintain thorough and timely documentation of all patient visits in accordance with agency and payer requirements Ensure accurate and current medication records are maintained Required Qualifications Licensed Practical Nurse (LPN) license Experience with OASIS data sets Proficiency in nursing skills Current CPR or BLS certification Preferred Qualifications Positive attitude and strong work ethic Self-motivated and compassionate approach to patient care Team-oriented mindset Strong productivity and time management skills Detail oriented with excellent organizational abilities Effective communication skills Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, pet insurance and employee discounts with preferred vendors.
