Job Description:\n\nDescription:\nPOSITION TITLE: Licensed Practical/Vocational Nurse\nDEPARTMENT: Nursing\nEMPLOYEE REPORTS TO: DON, CNO\nSUPERVISES: CNA\nFLSA STATUS: Non-Exempt\nEMPLOYMENT STATUS: Full-time, Part-time, Per Diem \n\nDESCRIPTION OF POSITION\nThis job description is a record of the essential functions of the listed job. The facility will make reasonable accommodations to otherwise qualified\nindividuals who are capable of performing the essential functions of the job with or without reasonable\naccommodation.\n\nPOPULATION SERVED\nThe position involves direct patient care for a population of patients between the ages of\n18 and older. Age specific experience and/or special training and/or expertise is\nrequired to serve this population.\n\nPOSITION SUMMARY\nUsing independent and interdependent judgment, the Licensed Vocational/Practical\nNurse (LVN/LPN) maintains the delivery of quality care by assuring the nursing care\nplan is followed. In collaboration with the physician, and the supervising RN, the\nLVN/LPN assures implementation of the assessment plan, evaluation, and follows up of\nthe plan of care for the patient population.Requirements:\nPOSITION QUALIFICATIONS\nThis position requires a valid temporary or permanent CA State Practical/ Vocational\nNurse License. The individual must have strong interpersonal skills, the ability to\nmanage multiple priorities, be dedicated to the provision of quality patient care, and use\nsound judgement in decision-making. BLS certification required upon hire, a valid\nACLS certification and IV & Blood withdrawal certification required within 120 days.\nAbility to maintain assigned work hours: Requires endurance to perform tasks over\nlong periods of work hours.\nThe duties and responsibilities are listed below. It is the expectation of the institution\nthat the employee be able to demonstrate competency in each area. It is the duty of the\ninstitution to provide orientation and performance evaluation in each area stated.\n\nDUTIES AND RESPONSIBILITIES\nI. RESPONSIBILITIES TO THE PATIENT\nA. Participates in an on-going systematic assessment of the physical,\nemotional, social, educational, and functional needs of the patient/family\nthrough interview, observation, and physical examination.\n\n Provides plans, coordinates or manages nursing care, nursing\n\nservices, and health education to patients/ residents.\nB. Collects assessment data and information to develop a nursing care plan\nwhich contains problem identification, nursing diagnosis, nursing orders,\napproaches, interventions, short/long term goals, and discharge planning.\n\n Develops nursing care plan utilizing nursing diagnosis\n Assesses safety and fall risk levels for patients\n\nC. Assures that medical orders are transcribed and processed accurately,\nand integrates the medical care plan into the provision of nursing care.\nD. Assures that medication administration is correct and according to the\nestablished nursing policies and procedures.\n\n Looks up unfamiliar medications\n Processes medication orders\n Administers medications as ordered\n Evaluates effectiveness of PRN medications\n Verbalizes process for self-administered medications\n Verbalizes patient education on identified medications\n Verbalizes standing orders\n\nE. Provides for personal and environmental safety in accordance with\nhospital and nursing policies and procedures\nF. Demonstrates skills in handling emergency and life-threatening situations\n\n Locates and uses crash cart items\n Calls Code Blue and Medical Emergency\n Initiates CPR\n Follows P&P and POLST\n\nG. Assures that documents in the medical record are complete, factual,\naccurate and timely.\n\n Documents materials and equipment used.\n Verbalizes method to noting orders.\n Documents discharge in chart.\n Initiates and updates care plan as appropriate.\n Provides discharge instructions.\n Completes interagency transfer form.\n\nH.Provides total nursing care including activities & daily living (ADL) to meet\npatient needs:\n\n Performs a complete head-to-toe assessment of each assigned patient\n\nper shift using proper documentation tools.\n\n Acts promptly on changes in patient condition, utilizing SBAR method\n Reviews lab and test results, notifying physician of abnormal and\n\ncritical results\n\n Performs wound care in accordance with policy, utilizing proper\n\ndocumentation tools.\n\n Performs basic ventilator management and trouble-shooting.\n\nII. RESPONSIBILITIES TO UNIT STAFF\n\nA. Provides direction and leadership in patient care to assigned personnel.\n\n Acts as a team leader and provides guidance to unlicensed personnel.\n Makes assignments for team.\n\nB. Prioritizes activities and plans for effective use of time for patient needs.\nC. Communicates the plan of nursing care for each patient through reports,\nconferences, rounds, and patient care records.\n\n Shift to shift report and shift huddle with aides\n\n 2. Delegates tasks to aides; monitors to make sure delegated\n tasks are completed appropriately.\nD. Demonstrates responsibility for the teaching of nursing assistants in areas\nof new techniques, policies and procedures.\n\n Preceptor\n\nE. Participates in the quality improvement activities of nursing.\n\n Attends appropriate meetings.\n\nF. Communicates identified learning needs of self and others to the DON.\nG. Assists the DON in the orientation of new staff.\nH. Adheres to established policies and procedures of the hospital and\nNursing Department.\nI. Actively participates on nursing committees.\nJ. Performs other duties as assigned or requested.\nIII. RESPONSIBILITIES TO SELF\nA. Evaluates own performance and seeks opportunities for growth and\nexpansion of skills\nB. Takes initiative in upgrading and maintaining skills necessary to provide\nquality care to provide quality care to patients and leadership to assigned\nstaff.\nC. Acquires continuing education sufficient to maintain licensure and\ncertification\n\n Maintains current nursing licensure\n Provides Human Resources and DON with a copy of current nursing\n\n license.\n\n Maintains required competencies\n Completes assigned program tracks assigned by DON/DSD.\n Completes appropriate level of Clinical Advancement within\n\n established timeframes\nD. Membership in appropriate professional organizations.\nE. Assumes responsibility for maintaining/enhancing physical health and\nalleviating stress\n\n Attendance\n\nF. Demonstrates competency in a variety of nursing skills as demonstrated\nand evaluated on at lease an annual basis.\n\n Attends annual skills fair\n\nG. Displays a “customer service” attitude toward patients, visitors, and other\nstaff members at all times.\n\n Demonstrates basic telephone skills (how to answer, place on hold,\n\n and transfer a call.)\n\n Utilizes intercom\n Demonstrates how to page physicians or others on call.\n\nH. Abides by all safety, infection control, and other guidelines as required.\n\n Follows guidelines for caring for patients in isolation\n Utilizes hand-washing techniques\n Cleans equipment between patient usage.\n Reports personal symptoms of suspected illness or contagious\n\n disease to the Employee Health Nurse.\n\n Demonstrated knowledge of universal blood and body fluid\n\n precautions\nI. Promotes an ethic of continuous quality improvement in all matters\nassociated with the position, and documents specific quality improvement\nimplementation.\nJ. Performs all other tasks and duties as may be assigned.\nK. Provides a level of competency to treat patients 18 and older.\n 1. Adults:\n\n Involves them with care planning by meeting any needs they\n\n identify during assessment\n\n Identifies their normal patterns of living and strives to attain them\n\n 2. Geriatric:\n\n Strives to obtain understanding by speaking distinctly and\n\n moving slowly.\n\n Involves family as much as possible\n\nIV. DRESSES AND PRESENTS SELF IN A PROFESSIONA MANNER AT ALL\n TIMES. ADHERENCE TO THE CORPORATE APPEARANCE\nPOLICY IS REQUIRED.\nV. WEARS HOSPITAL ISSUED NAMETAG AT ALL TIMES WHILE ON\n DUTY\nVI. Unsafe Conditions – Immediately reports and corrects, if possible, unsafe\nconditions or equipment.\nC. Workplace Standards – Compiles with relevant regulations, standards\nand policies governing safe workplace environment (OSHA, Accreditation,\netc.)\nD. Safety Techniques – Maintains current knowledge of all aspects of the\nfacility’s safety program by attending safety-related training as mandated\nupon hire and thereafter as required by facility.\nE. Modified Work – Accepts modified work assignments after receiving the\nphysician’s release to return to work following an accident/ injury.\nF. Follows Prescribed/Recommended Treatment – Follows the\nprescribed/recommended treatment given by treating physician(s) after an\naccident/injury.\nV. EDUCATION – Participates in required inservice and educational programs on\nan ongoing basis.\nVI. ATTENDANCE AND PUNCTUALITY – Follows all facility guidelines outlining\nstandards of attendance and punctuality. Responsible for reporting to and\ncompleting work at assigned times.\nUNIVERSAL PRECAUTIONS\nUniversal precautions will be observed in order to prevent contact with blood or other\npotentially infectious materials. Under circumstances in which differentiation between\nbody fluid types is difficult or impossible, all body fluids shall be considered potentially\ninfectious materials. All blood or other potentially infectious materials will be considered\ninfectious regardless of the status.
