Health and Wellness Director

Health And Wellness Director (LPN/LVN or RN) Cogir Senior Living, based in Scottsdale, Arizona. The Health And Wellness Director is an integral member of the management team and works closely with the Executive Director to ensure the health and wellness of residents. They perform initial and ongoing assessments, observe residents’ physical and psychosocial needs, coordinate with other departments, provide training and supervision to the clinical team, and manage day‑to‑day clinical operations while complying with applicable laws. Key Responsibilities Foster a resident-centered approach to planning and delivering services. Assess residents’ health, functional, and psychosocial status; initiate individualized service plans and manage care for each resident. Establish and maintain the medication program. Oversee hiring, promotions, disciplinary actions, and terminations of licensed nursing staff and care staff. Provide hands‑on training, supervision, and staff development. Communicate regularly with residents’ families regarding health care needs. Maintain current departmental policies, procedures, and licenses. Ensure resident records and charting are current and up to date. Represent the community in a positive manner to outside agencies. Candidate Qualifications Education and Certifications An LVN/LPN or RN nursing license in good standing. Associate degree or higher in nursing preferred; additional coursework in management is an asset. Current state-required certifications such as First Aid, CPR, and any other applicable credentials. Experience, Competencies, and Skills At least four years in geriatric nursing, including medication administration and pharmacy experience. At least two years in a supervisory/management role, including budgeting, staff development, training, and scheduling. Experience in Memory Care preferred. Outstanding organizational skills. Proficient computer literacy and knowledge of the Microsoft Office suite. Ability to work a flexible schedule, including weekends, holidays, and overtime; on‑call 24/7 capacity.
