Pediatric Home Health Nurse - Transitional Care (RN/LPN)

Pediatric Home Team (Transitional) Nurse – RN / LPN Location: Houston, TX | Bilingual Preferred (Not Required) Pay Range LVN: $30–$35/hour RN: $35–$42/hour Angels of Care Pediatric Home Health is hiring RN and LPN Pediatric Home Team (Transitional) Nurses to support medically fragile children and their families during critical transitions to home care. This role is ideal for nurses who are confident working with high‑acuity patients and skilled in trach, vent, G‑tube, and emergency management. What You’ll Do Provide skilled pediatric nursing 1:1 care in patients’ homes Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies Support hospital-to-home transitions for high‑acuity patients Educate and support parents, families, and caregivers Assist with discharge support and care coordination Train and support permanent field nurses during transitions Provide first‑shift and interim coverage as needed Collaborate with Clinical Care Managers and interdisciplinary teams Respond appropriately in emergency situations and changes in patient condition Qualifications Active Texas RN or LVN/LPN license Current CPR certification Ability to lift and assist pediatric patients as needed Experience with trach care and ventilator management High‑acuity pediatric patients experience (preferred) Emergency response management Pediatric home health experience (preferred) Schedule & Travel Full‑time position with guaranteed hours Flexible scheduling based on patient needs and nurse availability 5 working days per week On call and staffing coverage responsibilities as needed Local travel within approximately 30–40 miles Benefits Major medical, dental, & vision plans with generous coverage Group & voluntary life insurance 401(k) Paid time off Employee Assistance Program Supplemental insurance policies Patient Care Coordination Services Prescription Concierge Service Anytime Fitness gym discount Voluntary long‑term & short‑term disability Free life insurance Cellular discounts Health savings account Dependent care flexible spending account Wellness program & incentives Legal Shield & Identity Shield Flexible Spending Accounts (FSA) Annual car giveaway and many more!
