Patient Care Coordinator

What You'll Do: Welcome patients and families and provide a professional, supportive experience. Answer and manage patient calls, messages, and administrative inquiries. Schedule and coordinate new-patient and follow-up appointments. Assist patients with intake paperwork and practice processes. Manage appointment confirmations, cancellations, rescheduling, and no-shows. Coordinate medical-record requests, referrals, and secure faxes. Assist with administrative pharmacy communications, such as confirming prescription receipt or obtaining pharmacy information. Route clinical questions and medication concerns to the appropriate clinical team member. Maintain accurate administrative documentation in the electronic health record. Collect assigned patient payments and support front-office administrative processes. Maintain an organized, welcoming reception area. Support daily opening/closing workflows and securely close the office Monday through Thursday. Follow through on assigned patient requests rather than simply forwarding messages. Protect patient confidentiality and follow HIPAA and Lighthouse MindCare policies.Bilingual English/Spanish is required. Candidates must be comfortable communicating professionally with patients and families in both languages by phone, secure messaging, and in person. Warm, compassionate, and patient-centered. Professional and confident on the phone. Dependable and punctual. Highly organized and attentive to detail. Comfortable learning new technology and electronic health record systems. Able to manage multiple priorities without losing follow-through. Calm and respectful when working with frustrated or distressed patients. Able to recognize when a question needs to be escalated rather than guessed. Comfortable receiving feedback and learning new workflows. Able to work independently while remaining part of a collaborative team. Previous experience in a medical, behavioral-health, psychiatric, or other healthcare office is preferred, but we are willing to train the right candidate who demonstrates excellent judgment, professionalism, communication skills, and commitment to patient care.
