RN Care Navigator: Primary Care & Wellness Outreach

Mount Carmel Health System in Ohio is seeking an RN Care Navigator to coordinate patient care within the Ohio Nurse Practice Act. You will provide compassionate, team-based care across primary care settings in Columbus and surrounding locations. Responsibilities include Medicare wellness visits, outreach to patients, and seamless communication with providers using EPIC.
