Physical Therapist

Description This is a full time position supporting our Tanasbourne site. This position is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA and PT aide. Within 30 days of hire: National Provider BLS - American Heart Association Preferred qualifications: Master's Degree in Physical Therapy. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
