Four Seasons Nursing and Rehabilitation Center is seeking a passionate full-time Rehabilitation Transporter to join our team!

Job Responsibilities (not limited to the below):

·        Transporting of residents to and from the therapy department.

·        Informing nursing staff that a resident will be transported to Occupational/Physical therapy and inform nursing staff when a resident is returned to the unit.

·        Collecting equipment which needs repair when a resident is hospitalized or discharged.

·        Maintain Infection Control Standards for Sanitizing of Equipment and items utilized for Patient Care

·        Assists the licensed therapists in clerical and patient/resident related duties.  May involve computer skills.

·        Assists with maintaining an adequate stock of supplies and equipment.

·        Maintains adequate records indicting the reason a resident misses a scheduled therapy visit

·        Maintain confidentially of all resident care information.

·        Maintaining adequate records, indicating the reason why a resident misses a scheduled therapy visit.

·        Maintain professional conduct and language with staff and residents

·        Maintain integrity, honesty and respectfulness with staff and residents at all times

·        Accurately reviews full house equipment/device list and checks wearing schedules, condition of equipment and fit of device in order to directly communicate/refer to PT/OT staff for changes in status.

·        Ambulation of residents as prescribed PT eval/orders for levels of Assist and Cueing

·        Consistently confer with Medical Records and Nursing staff to prepare and organize Rehab clinical documentation required for MD Signatures

·        Reliably collect, organize, file and maintain Rehab medical documentation necessary for Preparation of AUDIT Files

·        Effectively and reliably adheres to resident’s weight bearing precautions and restrictions as prescribed and delineating by supervising PT/OT staff based on diagnosis.

·        Competently and Effectively perform Nursing Rehab program duties as prescribed by Primary OT Staff for safe ROM and donning/ doffing of splints/Equipment on residents.

·        Competently and effectively perform Nursing Rehab program duties as prescribed by Primary OT staff for safe self-care/hygiene assistance of resident in prep for therapy services.

·        Perform related duties, as required.

Job Qualifications:

• Graduate of an accredited high school or GED preferred.

• Experience in physical therapy or Rehab setting

• Knowledge of assistive devices for ambulation, patient transfer equipment (wheelchair, stretcher functions and safety) preferred.

Experience:

• Long term care: at least 1 year (Preferred)

Schedule: Monday- Friday 9am-5pm

Work Location: One location/Onsite
