Nurse Case Manager

Job Description Summary Nursing Case Managers strives to promote seamless transitions between different healthcare settings such as hospitals, rehabilitation facilities, and home care services. By coordinating communication and information sharing among providers, they help prevent gaps in care and reduce the risk of medical errors. Develop, document, and implement plans to provide appropriate resources addressing the social, physical, mental, emotional, spiritual, and supportive needs of the patient. Review criteria to assess and document the appropriateness of admission, continued stay, and level of care. Assist patients, their families, and physicians with post‑acute placement. Advocate for patients ensuring they receive the best care in a cost‑effective way. Refer cases that do not meet admission and/or continued stay criteria to the designated Physician. Educate patients and families about treatment options and health status. Perform additional assignments and tasks as assigned by Case Management Department Head. Work Requirements Will be required to be on call at times. Must follow directions and perform work under limited supervision according to department standards when no direction is given. Must be emotionally mature and able to function effectively under stress. Experience 1 year of case management and/or utilization review work experience preferred. Education & Licensure Graduate from an accredited school of nursing and a minimum of one year of work experience as a Registered Nurse required. Bachelor of Nursing degree preferred. Licensed as a Registered Nurse by the South Carolina Board of Nursing or a compact state. Case Management certification is preferred.
