• Shift Duration: 8-hour shifts
• Shift Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with evening and weekend on-call rotation
• Estimated Weekly Pay: $1,350 - $1,420
• Setting: Home Health and Hospice
• On-Site: Yes
• Productivity Expectation: 32 points per week

Job Requirements

• Active West Virginia LPN license
• At least 1 year of home health experience required
• Experience with HCHB strongly preferred
• Ability to manage a patient caseload with attention to quality, accuracy, and timeliness
• Strong documentation, charting, and communication skills
• Comfortable working independently and participating in an on-call rotation
• Ability to travel within the assigned service area

Responsibilities

• Deliver skilled nursing care according to the patient's plan of care while supporting dignity and independence
• Monitor vital signs and promptly report abnormal findings
• Administer medications, feedings, oxygen, and ostomy care as needed
• Carry out care routines established by other healthcare professionals such as PT, OT, and Speech Therapy
• Maintain infection control practices and report possible communicable diseases or infections
• Document all nursing actions, communications, progress, and outcomes accurately
• Order supplies, medications, and feedings to help ensure adequate availability
• Monitor incidents, complete reports in a timely manner, and notify appropriate personnel
• Assess individuals following incidents and ensure medical equipment remains in good working order
