That includes yours.You will report to the Associate Medical Director, and work at our clinic located at Phillips Hwy, FL.Why You'll Love Working HereTeam-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness.More Time with Patients: See fewer patients per day and spend more time delivering meaningful care.Supportive Culture: We foster a welcoming, inclusive environment where teamwork and growth are prioritized.Your Role & ResponsibilitiesYou will provide comprehensive care to seniors following clinical standards.You will collaborate daily with your care team to ensure coordinated, high-quality care.You will manage referrals, hospital/SNF coordination, DME, and home health services.You will document care with support from quality-based coders.You will participate in on-call rotation and support growth plans at your center.You will have experience with providing value-based care.You will work on-site.What We're Looking ForRequired:Master's degree in nursing with board certification.Active, unrestricted NP, or PA license in the state of Florida.Experience in value-based care, primary care or Medicare Advantage settings.Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbersFamiliarity with HEDIS quality indicators and Medicare guidelines.Scheduled Weekly Hours: 40Pay Range: $123,800 - $170,300 per yearThis job is eligible for a bonus incentive plan.
