Responsibilities
- Develop, implement, and evaluate health plan care management program lines to support members
- Promote continuity of care and cost-effectiveness through whole-person in-patient and out-patient management
- Coordinate care integration, case management, utilization review, and comprehensive care planning
- Collaborate with multidisciplinary teams to optimize member outcomes and ensure adherence to care plans
- Monitor outcomes and adjust care plans based on member needs, clinical progress, and performance metrics

Requirements
- Active Registered Nurse license
- Ability to work in a hospital-based, in-person setting
- Strong communication and collaboration skills
- Knowledge of care management, utilization review, and care planning processes
