Role Overview The RN Care Manager provides patient-centered care coordination, assessments, and education to support optimal health outcomes. Key Responsibilities Conduct comprehensive patient assessments, develop individualized care plans, coordinate referrals and services, facilitate care transitions, and monitor patient progress. Serve as a liaison among patients, families, healthcare providers, and community resources, ensuring timely treatments and adherence to regulations. Maintain accurate electronic health records and participate in quality improvement initiatives. Qualifications & Skills Bachelor's or Master's degree in Nursing, current RN license in Illinois, and at least 2 years of clinical experience, preferably in acute or hospital settings. Preferred certifications include CCM and CPR/BLS. Strong communication, collaboration, and organizational skills are essential. Additional Requirements Flexibility to cover major winter and summer holidays, and proficiency in healthcare regulations and standards.
