Registered Dietitian

Overview Centers Health Care is seeking a Registered Dietitian to join our Healthcare & Medical Services team. In this role, you will assess nutritional needs, develop individualized care plans, and collaborate with interdisciplinary teams to support patient health outcomes. Responsibilities Conduct comprehensive nutrition assessments for patients, including dietary history, anthropometrics, clinical data, and nutrition-focused physical exams. Develop and implement individualized nutrition care plans based on medical conditions, cultural preferences, and evidence‑based guidelines. Provide medical nutrition therapy for a variety of conditions such as diabetes, cardiovascular disease, renal disease, GI disorders, obesity, and malnutrition. Document all assessments, interventions, and patient progress accurately and promptly in the electronic health record (EHR). Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team during rounds and care conferences. Provide individual and group nutrition counseling and education to patients, residents, and families. Monitor and evaluate patient response to nutrition interventions and adjust care plans accordingly. Participate in menu planning and review to ensure compliance with clinical, regulatory, and dietary standards. Support oversight of food service operations to help ensure adherence to sanitation, food safety, and infection control policies. Contribute to quality improvement initiatives and development of nutrition‑related policies, protocols, and education materials. Stay current with clinical nutrition research, guidelines, and best practices through continuing education and professional development. Promote a patient‑centered, culturally sensitive, and inclusive approach to nutrition care. Clinical nutrition assessment and medical nutrition therapy (MNT). Qualifications Knowledge of disease‑specific nutrition guidelines (e.g., diabetes, renal, cardiac, GI, oncology). Nutrition‑focused physical examination (NFPE) skills. Proficiency with electronic health records (EHR) documentation. Menu planning and nutrient analysis. Understanding of regulatory and accreditation standards (e.g., CMS, Joint Commission, long‑term care regulations). Food safety, sanitation, and infection control principles. Patient and family education and counseling skills. Interdisciplinary teamwork and communication. Data interpretation including lab values and anthropometrics. Time management and case load prioritization. Quality improvement and outcome monitoring. Cultural competence in nutrition counseling. Registered Dietitian (RD/RDN) credential from the Commission on Dietetic Registration (CDR). State licensure as a Dietitian/Nutritionist (as required by state regulations). Completion of an ACEND‑accredited dietetics program and supervised practice/internship. Current CPR/BLS certification (preferred). Experience with electronic health records (EHR) systems (preferred).
