Physical Therapist (PT) - Adult NeurorehabilitationLocation: Johns Creek, GA Job Type: Full-Time / Part-Time Work Setting: On-Site | Outpatient RehabilitationPosition OverviewWe are seeking dedicated and compassionate Physical Therapists (PTs) for full-time and part-time opportunities in Johns Creek, GA. This role focuses on providing specialized neurological rehabilitation to adults with brain injuries, neurological conditions, and developmental disabilities.The ideal candidate will evaluate patient mobility, develop individualized treatment plans, and deliver evidence-based interventions to improve functional movement, independence, and quality of life.Key ResponsibilitiesClinical Evaluation and Care PlanningConduct comprehensive physical therapy evaluations, including patient history, systems reviews, and standardized assessments.Identify movement dysfunctions and develop individualized plans of care.Focus on transfer training, gait training, bed mobility, and community mobility.Monitor patient progress, reassess outcomes, and modify treatment plans as needed.Develop appropriate discharge plans to support continued recovery and independence.Treatment and RehabilitationProvide therapeutic interventions to improve mobility, reduce pain, restore function, and prevent long-term disability.Recommend and train patients and families on adaptive equipment, compensatory strategies, and home modifications.Develop cardiovascular conditioning and individualized fitness programs.Adapt vocational, recreational, and daily living activities to meet patient needs.Educate caregivers and family members on techniques that support rehabilitation goals.Interdisciplinary Collaboration and DocumentationCollaborate with occupational therapists, speech-language pathologists, case managers, nursing staff, and other healthcare professionals.Participate in interdisciplinary meetings to coordinate patient care and treatment goals.Maintain accurate clinical documentation, including evaluations, treatment notes, and progress reports.Ensure compliance with applicable state regulations, HIPAA, and relevant accreditation standards.Coordinate care with physicians and other healthcare providers.Maintain professional licensure and participate in continuing education.Qualifications and RequirementsGraduate of an accredited Physical Therapy program.
