Overview Licensed Clinical Social Worker position with NHC Home Care Merritt Island. Pay: $40 - $47 per regular visit. Pay is based on experience. Responsibilities Provide clinical social work services to evaluate the social and emotional needs of patients related to their medical condition. Offer counseling and support based on the evaluation. Supervise and coordinate social work services within the home care team to support patient care and discharge planning. Assess social and emotional factors to minimize impact on the patient’s health and ability to cope with daily living problems; help patients and caregivers understand, accept, and follow medical recommendations. Identify resources, including caregivers and community agencies, to assist the patient in resuming life in the community. Participate in the development of the plan of care with the providers and home care team. Qualifications Master’s Degree from a school of social work accredited by the Council on Social Work Education. One year of social work experience in a health care setting is required. State-specific requirements: Florida: current license as a Licensed Clinical Social Worker (LCSW), pursuant to Chapter 491, F.S.
