Responsibilities: \n You will coordinate individualized care plans that align with each patient's clinical, emotional and psychosocial needs — ensuring timely access to services and support across the continuum of care. Partnering with providers and interdisciplinary teams, you will facilitate communication, address barriers to treatment and help patients and families navigate complex healthcare decisions. You’ll connect families with community, financial and supportive resources and programs while promoting informed decision-making and continuity of care. In addition, you will contribute to program development through quality improvement initiatives, policy enhancement, staff education and mentorship in perinatal and bereavement care practices.
