Job DescriptionJob DescriptionResponsible for coordinating case management services for unaccompanied children in care under the direction of the Lead Case Manager. This includes coordinating timely referrals to external care providers and consistent communication with relevant stakeholders on referral status, ensuring consistent care coordination communication between medical and behavioral health staff internally and externally, coordinating timely referrals to child advocate services as needed, ensuring that the appropriate health interpretation and translation services are available, and ensuring the coordination of the appropriate post-release services that include the continuity of health care services.Case Manager Duties and ResponsibilitiesMake case studies for the purpose of assessing problems and determining appropriate types and methods of treatment.Investigate and provide services and referrals for clients where their physical or emotional welfare is involvedWork with cases involving neglect, abuse, emotional or behavioral problems, physical/mental disabilities or other health conditions affecting a client's personality, family relationships, and child or in-home care arrangements.Develop intensive long or short-term treatment plans to improve or restore individual or family functioning.Conduct thorough psychosocial assessments on families.Develop person-centered, strength-based case plans.Drive frequent long distances to conduct field visits in order to carry out service plans, observe, and assess client needs, and provide information and social work servicesMaintain up-to-date records of all aspects of case management and prepares reports and correspondence.Case Manager Requirements and QualificationsBachelor's Degree in the behavioral sciences, human services or social services fields.Child welfare and/or case management experience is strongly encouraged.Experience in case planning, case management, and fieldwork in a public or private agency as a Case Manager or closely related job title.Possesses superb written and spoken communication skillsExcellent interpersonal skills with colleagues, community leaders, policymakers, and othersExcellent time management skills; organized and able to prioritizeMotivated to take on additional community involvement projects and solve problemsComfortable in a fast-paced environment with multiple casesAble to organize and manage large amounts of files, schedules, dates, and informationSelf-directed and able to work without supervisionComfortable with building personal relationships and dedicated to helping othersEmpathetic and supportive with mentorship and leadership skillsOtherBilingual - Spanish/English requiredBe at least 21 years of ageSuccessful completion of background checkBe able to show proof of immunity to vaccine-preventable diseases transmitted by the respiratory route [i.e.: MMR, Flu, Varicella, TB, etc.]Valid California Driver’s License, reliable personal vehicle, and active auto insurance for required travel.Flexibility to maintain 24/7 on-call availability as needed and provide coverage during evenings, weekends, or holidays based on case requirements.\nCompany DescriptionWe are a foster care and adoption agency including Transitional Foster Care & Intensive Services Foster Care.Company DescriptionWe are a foster care and adoption agency including Transitional Foster Care & Intensive Services Foster Care.
