About the Position

We are seeking a  Travel Physical Therapist (PT) for a Home Health assignment covering the Denton, Texas area . The Physical Therapist will provide skilled therapy services to patients in their homes, helping improve mobility, strength, balance, safety, and overall functional independence.

This position offers a consistent Monday–Friday schedule with 40 guaranteed hours per week . New graduates are welcome to apply.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized plans of care based on patient needs, diagnoses, and functional goals.
• Provide therapeutic exercises, strengthening, gait training, balance training, and functional mobility interventions.
• Assist patients with improving transfers, ambulation, mobility, and overall independence.
• Assess home safety and recommend appropriate assistive devices or modifications when needed.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on exercises, mobility techniques, safety, and home programs.
• Complete accurate and timely clinical documentation.
• Communicate and collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.
• Travel throughout the Denton, TX area to complete scheduled patient visits.
• Provide compassionate, patient-centered care in the home environment.

Requirements

• Graduate of an accredited Physical Therapy program.
• Active Physical Therapist license in Texas or ability to obtain one.
• New graduates are welcome to apply.
• Strong clinical, communication, organizational, and documentation skills.
• Ability to work independently in a Home Health environment.
• Strong time-management skills and ability to manage a daily patient schedule.
• Ability and willingness to travel between patients' homes throughout the assigned territory.
• Current BLS certification may be required.
