We are currently seeking a compassionate and experienced On-Call RN to join our team on a full‑time basis. This position would work 7 days on / 7 days off. What You’ll Do Provide skilled nursing care and support to hospice patients during on‑call hours Respond promptly to patient needs, triage calls, and make in‑home visits as necessary Collaborate with the interdisciplinary team to ensure coordinated care Support patients and families through compassionate communication and clinical expertise Maintain Current Registered Nurse (RN) licensure in the state(s) of practice in good standing, with at least one year of experience under license Previous hospice or home health experience strongly preferred Work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership Work closely with patients on a 1‑to‑1 level but receive strong support from a patient care team Work independently from a day‑to‑day basis with activities that include phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations Manage time effectively to schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns Learn an electronic medical records system Be willing to drive with reliable transportation, a valid driver’s license, and auto insurance, to travel across the service area #J-18808-Ljbffr
