Assignment Details

1 year experience as LPN required.

Requirements

• New York LPN Licensure
• BLS certification (AHA or RQI)

Responsibilities

• Collect continuous data related to resident's physical and psychosocial condition and communicate findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans based on nursing diagnoses to meet resident outcomes, involving residents and families in the process.
• Recommend care plan revisions based on resident outcomes and collaborate with the healthcare team to ensure comprehensive care.
• Maintain current knowledge and competency in nursing practice; participate in mentoring and orientation of staff.
• Provide leadership within the nursing practice under RN supervision.

Compliance / Notes

Occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks may involve potential contact with blood, body fluids, or tissues.
