Position Summary:
The Resident Wellness Coordinator/Scheduler supports the day-to-day resident experience and the flow of care coordination at Bellamy Assisted Living, with a special focus on residents living with dementia, psychiatric/behavioral health needs, and those on hospice or near end of life .
This role serves as a central hub for residents, families, hospice partners, and the care team—coordinating appointments, assisting with orders and lab slips, tracking hospice-related communication, and supporting families through serious illness and end-of-life transitions. All clinical-related tasks are performed under the direction of the licensed nurse and within MNA scope of practice.
Essential Duties and Responsibilities:
Nursing Department Schedule
Coordinates and tracks medical, psychiatry, neurology, counseling and hospice-related appointments and visits.
Gathers, organizes and routes provider orders, medication orders, hospice orders and lab slips under the direction of nursing.
Maintains organized logs and tracking tools for upcoming and completed appointments, hospice visits, outstanding orders and lab results, and related follow-up items.
Serves as a non-clinical point of contact for residents and families regarding routines, appointments, hospice visits and general questions.
Builds and maintains professional, supportive relationships with residents and families, including those coping with dementia, psychiatric conditions and end-of-life situations.
Collaborates with hospice providers to coordinate visit schedules, family meetings and communication, under the direction of nursing and administration.
Observes and promptly reports changes in resident behavior, mood, participation or comfort to nursing and leadership.
Participates in care plan, behavior plan and hospice/end-of-life care meetings as requested.
Supports implementation of non-pharmacologic and comfort-focused approaches developed by the clinical and hospice team.
Partners with Life Enrichment to ensure activities and events are dementia-friendly, psych-sensitive and respectful of hospice and end-of-life needs.
Identifies residents who may be isolated, overwhelmed or withdrawing and helps connect them to appropriate activities or gentle engagement.
Assists with resident and family communications, including letters, notices, appointment reminders and event information.
Maintains up-to-date provider and partner lists, including primary care, psychiatry, neurology, hospice agencies and counseling providers.
Assists with phones, greeting visitors and supporting a calm, welcoming environment at the front desk as needed.
Supports resident and family satisfaction initiatives, including surveys and follow-up as directed by leadership.
Qualifications:
MNA (Medication Nursing Assistant) certification strongly preferred.
Experience working with dementia/Alzheimer’s and psychiatric/behavioral health populations preferred.
Hospice and end-of-life experience preferred.
CDP (Certified Dementia Practitioner) and CARES dementia training/certification preferred.
Prior experience in assisted living, memory care, long-term care, hospice-supportive roles or a related healthcare setting preferred.
Strong organization and follow-through with appointments, orders, labs and documentation.
Excellent communication and interpersonal skills with residents, families, providers and staff.
Ability to remain calm, patient and professional with residents who are confused, anxious or nearing end of life.
Comfortable using basic technology, including email, electronic records, calendars, fax and scanning.
