Located in San Jose, CA, Creekside Post-Acute is a 130-bed skilled nursing and rehabilitation facility with a 5-star Quality Measures and Overall Rating. We are committed to providing compassionate, high-quality care in a supportive environment.

We are seeking a dedicated and compassionate  Certified Nursing Assistant (CNA) to join our nursing team. New graduates and newly certified CNAs are welcome to apply!

Responsibilities

• Assist residents with daily living activities, including bathing, dressing, grooming, and feeding.

• Help residents with transfers, mobility, and repositioning while maintaining safety and comfort.

• Take and record vital signs as directed.

• Observe residents and report changes in condition to licensed nursing staff.

• Document care provided according to facility policies.

• Provide companionship and emotional support to residents.

• Follow infection prevention and safety procedures.

• Collaborate with nursing staff and other healthcare team members to provide quality resident care.

• Protect resident dignity, privacy, and confidentiality.

Qualifications

• Current and active California CNA certification.

• Newly certified CNAs are welcome to apply.

• Previous skilled nursing facility, long-term care, or senior care experience preferred.

• Strong communication and interpersonal skills.

• Ability to work effectively as part of a team.

• Compassionate, dependable, and resident-focused.
