Description

• Conduct thorough evaluations of patients in inpatient and outpatient environments and curate tailored treatment plans
• Deliver physical therapy treatments with a focus on mobility, strength, balance, and functionality
• Engage with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Educate patients and their families about therapeutic objectives, discharge strategies, and at-home exercise routines
• Maintain meticulous documentation of evaluations, treatments, and patient progress according to facility standards and regulations
• Take an active role in care team rounds and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited program

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are a bonus but not mandatory
