Family Care Coordinator / Columbus, GA - $3,000 Sign-on-Bonus Job Category: Consent, Referral, Vascular Placement Requisition Number: FAMIL001921 Employment Type: Full Time Posted: April 7, 2026 Location Columbus, GA Responsibilities Coordinate Family Care Coordinator call schedule and collaborate with vascular coordinator staff. Handle referral, pre‑authorization and authorization tasks: evaluate referrals after brain death, determine suitability, verify next‑of‑kin, notify first‑call vascular coordinator, conduct physical assessment, review hospital chart, discuss donor information with personnel, develop family plan, and when appropriate, approach family members to obtain authorization using approved consent and medical history forms. After authorization, provide family with information, notify first‑call vascular coordinator of outcome, introduce the vascular coordinator to family members, coordinate follow‑up with family, administer, and handle Hospital Development Liaison follow‑up, review and co‑sign family letters when appropriate. Initiate donor chart, write beginning orders, obtain necessary blood samples and nodes for serology testing and HLA tissue typing. Participate in a 24/7 call schedule rotation and respond to emergency back‑up call coverage. Travel as required by automobile or air travel. Qualifications Minimum of two years in a related field with experience in a medical or social services environment. Clinical experience preferred; prior roles such as Clinical Counselor, Bereavement Counselor, Healthcare Advocate, Funeral Director, Social Worker, Case Manager, or Family Counselor considered an advantage. Strong verbal communication skills. Current driver license with a good driving record. Collaborator who thrives in a mission‑first environment.
