Whole Family Health Center is seeking a compassionate and experienced Advanced Practice Registered Nurse (APRN) to join our medical team.

In this role, you will provide comprehensive, patient-centered care while working collaboratively with physicians, nursing staff, and other members of the healthcare team. What You'll Do

As an APRN at Whole Family Health Center, you will:

• Perform comprehensive, age-appropriate histories and physical examinations for patients with acute, chronic, and complex healthcare needs.
• Assess patient health status, establish differential diagnoses, and develop individualized plans of care.
• Order and interpret appropriate laboratory, diagnostic, therapeutic, and radiographic studies.
• Prescribe appropriate pharmacologic and non-pharmacologic treatments.
• Implement and monitor interventions to help patients regain or maintain optimal health.
• Evaluate patient progress and modify treatment plans as appropriate.
• Educate patients and families and support informed healthcare decision-making.
• Facilitate patient transitions within and between healthcare settings when necessary.
• Accurately and promptly document assessments, progress notes, treatment plans, and other required information in the Electronic Health Record (EHR).
• Provide clinical instruction and guidance to nursing staff as appropriate.
• Maintain current knowledge of clinical practices, healthcare processes, and applicable standards of care.
• Provide coverage at other clinic locations occasionally, as needed.

These responsibilities closely reflect the clinical duties in WFHC's existing APRN job description, including histories and physicals, diagnostic testing, prescribing, patient education, EHR documentation, and occasional travel between clinics.

What We're Looking For

Education & Licensure

• Graduate of an accredited school of nursing.
• Graduate of an accredited Nurse Practitioner program.
• Current Advanced Practice Registered Nurse (APRN) license .
• Current Nurse Practitioner certification in the applicable specialty area, if required.
• Current Basic Life Support (BLS) provider certification.
• Eligible for hospital privileges.

Experience

• Three (3) or more years of Nurse Practitioner experience preferred
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent verbal and written communication skills.
• Ability to practice independently while collaborating effectively with physicians and other members of the healthcare team.
• Spanish and/or Creole language skills are a plus.
• Level 2 background clearance required.
