Reports to: Clinical Manager

Essential Job Functions and Responsibilities:

• Provides physical therapy services to patients according to a written physician's plan of care period
• Follow conditions of participation
• Work within the scope of Physical Therapy
• Coordination of patient care
• Performs other duties as assigned

Position Qualifications:

• Licensed to practice as a physical therapist with through the state
• Preferred 2 years of experience as a Physical Therapist
• No Home Health experience is required
• Possesses and maintains current CPR Certification
• Driver License and Auto Insurance is required
