Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning. MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license BLS 2 years case manager experience in an acute hospital setting Acute hospital experience in specialty during the past year with no more than a 90-day break Knowledgeable in Meditech, Midas, and Interqual.
