We are seeking an experienced Hospice RN to provide compassionate, patient-centered care in the home setting.

**Requirements:**
* Active Vermont RN License
* Home Health experience required
* OASIS experience required
* Case Management experience required
* BLS Certification required
* Minimum 3 year of Hospice/Home Health RN experience
* Homecare Homebase (HCHB) experience preferred

**Assignment Details:**
* 16-week contract
* Monday–Friday Days
* Some weekend coverage required
* 36 guaranteed hours/week
* Holiday coverage required
* Mileage reimbursed at IRS rate between patient visits

**Responsibilities:**
* End-of-life and hospice care
* Pain and symptom management
* OASIS documentation and case management
* Collaboration with physicians and interdisciplinary care teams
