We are seeking an experienced and compassionate Travel Physical Therapist (PT) to join our Home Health team. In this role, you will provide high-quality rehabilitation services to patients in their homes, helping them improve mobility, regain independence, and achieve their functional goals. This position offers the opportunity to work independently while making a meaningful impact throughout the Coastal Bend region. Coverage Area

We are seeking an experienced and compassionate Travel Physical Therapist (PT) to join our Home Health team. In this role, you will provide high-quality rehabilitation services to patients in their homes, helping them improve mobility, regain independence, and achieve their functional goals. This position will provide care throughout:
- Corpus Christi
- Portland
- Taft
- Ingleside
- Aransas Pass
- Rockport
- Sinton
- Odem
- Beeville
- Mathis
- Robstown
- Day shifts
- Weekly Hour Guarantee: 32 hours
- Preferred weekend rotation: 1 weekend every 6 weeks
- Expected productivity: 30 points per week
- Perform Start of Care (SOC) assessments for Home Health patients
- Complete comprehensive physical therapy evaluations
- Develop and implement individualized treatment plans
- Perform patient reassessments and discharge evaluations
- Document patient care accurately and timely in accordance with agency, state, and federal requirements
- Collaborate with nurses, physicians, case managers, and interdisciplinary team members to coordinate care
- Educate patients and caregivers on treatment plans, safety practices, and home exercise programs
- Maintain compliance with all Home Health regulations and quality standards
- Current Physical Therapist (PT) license
- Recent Home Health Physical Therapy experience
- Strong clinical assessment and treatment planning skills
- Ability to work independently and effectively manage a travel schedule
- Valid driver's license and reliable transportation
