Licensed Practical Nurse - Salem Primary Care

Job Title

Starting Wage Range: $36.01/hr. - $46.31/hr.

Essential Functions:

• Assists medical professional staff in providing medical care to patients of the medical
• Assists in the management of resident patient services; adhering to regulatory principles and the oversight of policies and procedures for the care of the elder and disabled
• Functions under established routines for urgent care services to provide nursing care within scope of practice.
• Obtains patient intake information pertaining to visit to include chief complaint, vital signs, and

• Administers and documents injections per written/verbal order to include immunizations, allergy injections, other therapeutic injectable medications, and assesses patient response.

Education, Experience, and Licenses:

• Requires a current Oregon LPN
• A minimum of three (3) years' experience as a Licensed Practical Nurse with a minimum of one
• (1) year experience in a clinical setting preferred.

Driving Requirements:

• Employees may be required to drive for work-related purposes and must maintain a valid Oregon
• Driver's License (ODL), meet company vehicle insurance requirements, and provide proof of personal vehicle insurance.
