In this general pharmacy role, the Pharmacist ensures safe and effective medication use across diverse patient populations in community or health-system settings. The position emphasizes collaboration with clinicians to optimize therapeutic outcomes and maintain medication safety.

The Pharmacist dispenses medications, counsels patients, and reviews orders for accuracy, drug interactions, and appropriate dosing as part of the care team.

Responsibilities

• Review and verify prescription orders for accuracy, safety, and appropriate dosing.
• Counsel patients and caregivers on medication use, dosing, side effects, and adherence.
• Prepare, dispense, and confirm medications per policy while ensuring availability and accurate labeling.
• Monitor for adverse drug events and drug interactions, escalating concerns to the care team as needed.
• Collaborate with prescribers and other clinicians to optimize therapy plans and address drug shortages or shortages.
• Document interventions and medication histories in the electronic health record to maintain a complete record of care.

Qualifications

Education:

Doctor of Pharmacy (PharmD) from an ACPE-accredited program.

Licensure:

Current pharmacist license to practice in the United States and state-specific licensure as required.

Certifications:

• Board Certification in Pharmacotherapy (BCPS) or equivalent; preferred
• Immunization certification in accordance with state law

Experience or Skills:

Strong medication knowledge, excellent communication, and ability to work in a multidisciplinary team with proficiency in medication safety and pharmacovigilance.

Experience

• Experience providing medication therapy management and patient counseling in hospital or community settings
• Experience using electronic health records and pharmacy information systems
• Experience participating in multidisciplinary rounds or care planning meetings
• Experience with adverse event reporting and quality improvement initiatives

Preferred

• Experience in a fast-paced clinical environment such as emergency, ICU, or ambulatory care
• Immunization program administration experience and strong communication skills
• Ability to work flexible shifts to support coverage

Work Environment

The role operates in hospital, health-system, or community pharmacy settings with a multidisciplinary team of clinicians. The pace varies with patient volume and clinical demands, and documentation is completed in the electronic health record. Some on-call coverage may be required.

Physical demands include standing for extended periods and handling medications and supplies in a safe, compliant manner.
