divh2Job Title/h2pRN (Registered Nurse) Home Care/ph3Responsibilities/h3pProspective Candidate Pool We are accepting applications to build a pool of prospective candidates for anticipated future openings./ppWe are recruiting for opportunities in the Denver Metro area including Aurora, Denver and Lakewood with varying schedules./ppThe RN (Registered Nurse) Home Care provides skilled nursing care in accordance with InnovAge policies and all applicable regulations to participants and performs related administrative tasks for InnovAge. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties./pulliTriages participant and staff phone calls to determine priority of care and routes accordingly./liliSchedules outside appointments for participants when necessary./liliAssists in the examination, testing and treatment of participants./liliCompletes certification/recertification orders and discharge summaries/liliImplements orders written by primary care physician(s)./liliInstructs participants and family regarding medications and treatment instructions./liliSchedules specialist and other appointments for participants and assists in hospital admissions./liliPerforms general nursing care on participants including administering prescribed medications and treatments./liliDocuments all necessary information and maintains participant medical record(s) and fulfills agency charting and reporting requirements./liliAdministers medications, provides training in self-administration of all non-scheduled medications and monitors medication compliance./liliProvides health counseling to participants and their families on an individual or group basis./liliAssists in the examination, assessment, evaluation testing and treatment of participants./liliObserves, records, and reports participants condition and reaction to drugs and treatments to physicians/liliProvide nursing care to clients in their place of residence./liliAssess and re-assess participants to determine individual needs and to ensure the least restrictive environment./liliAssesses participants to determine change in condition that necessitates a change in level of care and coordinates the plan of care with Home Care resources./liliFurnishes those services requiring substantial specialized nursing skill./liliInitiates appropriate preventive and rehabilitative nursing procedures./liliPrepares clinical and progress notes for each client visit and summaries of care conferences for clients in a timely manner as per Agency policy./liliConducts client care conferences on clients assigned to his/her care./liliParticipates in peer review and quality management as assigned./liliParticipates in utilization review of medical records as assigned./liliProcesses orders and notifies physicians of client needs and changes in condition. Completes certification/recertification orders and discharge summaries./liliDetermines the amount and type of nursing needed by each individual client./liliEnsure that quality care is delivered by home care team to site participants via periodic participant/caregiver inquiry, intermittent review of care plan, and trending of data from various reports/supervisory visits in collaboration with the Home Care Manager./liliRefers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker for those clients requiring their specialized skills./liliSupervises the LPN every 30-days and supervises the HHA/CNA every 14-days (or per regulatory requirements) and teaches other nursing personnel./liliAdditional work functions and duties may be assigned/li/ulh3Required/h3ulliAssociates degree in nursing/liliCurrent State issued Registered Nurses License/liliCurrent CPR certification or the ability to obtain CPR certification/liliRequire personal transportation, current state issued drivers license, good driving record and auto insurance as required by law./lili1+ year working with the frail or elderly/li/ulh3Preferred/h3ulliBachelors degree in nursing/liliCertification as a Gerontological Nurse/lili3 years of health care experience with emphasis in geriatrics/liliBi-lingual/li/ulh3Benefits/h3pInnovAge is dedicated to empowering seniors to live independently, allowing them to age in their own homes and communities safely.
