Job Description
Title: Occupational Therapist - Rehabilitation

Schedule: Days 5x8

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: Not specified

Requirements:
- Graduate of an accredited Occupational Therapy program.
- Current Occupational Therapist license in the state of practice.
- Prior SNF or geriatric rehabilitation experience preferred.
- Strong clinical reasoning, communication, and organizational skills.

Roles & Responsibilities:
- Evaluate residents' functional abilities, including ADLs, mobility, cognition, and psychosocial factors.
- Develop and implement individualized treatment plans based on evaluation findings and physician orders.
- Provide direct occupational therapy services in accordance with state and federal regulations and facility policies.
- Monitor and document resident progress, adjusting treatment plans as necessary to meet goals.
- Participate in care planning meetings and collaborate with the interdisciplinary team.
- Educate residents, families, and caregivers on strategies to promote safety and independence.
