RN Case Manager

We seek a highly skilled and experienced Registered Nurse with at least 2 years of acute care experience to serve as a Case Manager. The role involves providing skilled nursing care, patient assessments, medication administration, treatment planning, and collaborating with interdisciplinary teams. The nurse will support patients across various care settings, ensuring effective communication and coordination of care. This position requires working every other weekend and the ability to float between units as needed. Responsibilities:

• Develop and implement treatment plans in collaboration with the interdisciplinary team.
• Manage case coordination and patient care management activities.
• Communicate effectively with patients, providers, and team members to ensure quality care.
• Float between like units based on staffing needs.
• Complete accurate and timely charting using Epic electronic medical records.
• Work every other weekend as required.

• Active Registered Nurse license in the state of practice.
• Minimum of 2 years acute care nursing experience.
• Clinical assessment, medication administration, and treatment planning skills.
• Strong interdisciplinary team collaboration and communication skills.
• Proficiency in electronic medical record charting (Epic preferred).
• Bachelor's degree in Nursing or related field.
• Driver's license, car insurance, and reliable automobile for field work if applicable.

• Experience in case management or care coordination roles.
• Previous experience working in multiple acute care units.

• Required to work every other weekend (EOW).
• No on-call responsibilities.
• Must be able to float between like units based on clinical needs.
• Free parking provided on first day; assignment parking arranged during orientation.
• Facility located near multiple state and national parks with resources available to assist with housing.

• Work schedule includes every other weekend shifts.
• Day and/or night shifts assigned based on unit needs and staffing.
