Key Responsibilities\n

• Develop and implement personalized support packages tailored to each client's unique needs to help them manage difficulties and overcome dependencies.

• Conduct thorough assessments by interviewing service users to evaluate their current psychological condition, strengths, weaknesses, and individual needs.

• Set specific, measurable goals for each client, focusing on interventions that enhance their coping mechanisms.

• Regularly monitor and evaluate client progress, updating treatment plans as necessary to ensure optimal outcomes.

• Provide counseling and valuable information to clients, guiding them in making informed decisions about their care during individual sessions.

• Maintain meticulous and up-to-date records of client interactions, documenting progress and reporting status as needed.

• Attend case conferences to discuss treatment progress and collaboratively refine approaches with the multidisciplinary team.

• Serve as a key worker for assigned clients, coordinating with external community services to support ongoing recovery and wellness initiatives.

Patient Population / Client Focus\n

As a Clinical Social Worker in our hospice center, you will primarily work with terminally ill patients and their families, addressing complex emotional and psychological needs. You will assist clients in coping with issues related to end-of-life care, grief, loss, and other mental health challenges. Qualifications\n

• A Master’s degree in Social Work (MSW), Psychology, or Sociology is required.

Compensation and Benefits\n
