RN - Hospice Profession: Nursing Specialty: Hospice Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and Oasis Description: Provides home nursing care according to physician's orders.Coordinates and directs the provision of nursing care following agency policies and procedures.Responsible for competent application of the nursing process.Supervises and evaluates Home Health Aides and Licensed Practical Nurses.Performs home visits and attends periodic office meetings.Must have reliable transportation, a current driver's license, and agency-required liability insurance.Average number of patients per day is 5-8.Handles patient care visits, documentation, and travel efficiently.Types of cases include Start of Care, Wound Care, and Follow-up Visits.Schedule allows for variability in weekend requirements depending on location.Is required to be on-call for back-up support twice a week.Must be willing to travel up to 75 miles from the designated care center.Mileage is reimbursed at the IRS standard rate.Documentation must be completed within 24 hours of each appointment.There are no guaranteed hours, but the clinician must hit weekly productivity points.Participants may have the opportunity to float to other clinics if necessary.All candidates must understand the expectations regarding patient assignments and scheduling. #J-18808-Ljbffr Cynet HealthStaff
