Providence Center Residential Case Manager Atwood Street

The Providence Center Residential Case Manager Atwood Street will provide clinical case management services in order to facilitate program goals and objectives using a quality care approach. Identify client needs with a person-centered approach to address not only mental health symptoms but co-occurring disorders. Working with consumers to achieve the highest level of independence possible.

Duties and responsibilities include initiating treatment and advocating for the consumer within the network of care. Negotiating and scheduling appointments for consumers within The Providence Center and with outside agencies. Assisting in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. Developing professional relationships with Day Program staff. Providing and maintaining a home atmosphere. Maintaining and promoting family contacts and relationships. Completing recordkeeping in accordance with Health Information Services and other Center requirements and abiding by Center confidentiality policy. Completing required interagency and other necessary paperwork. Completing shift activity logs. Completing all required documentation with appropriate format. Completing a clear, specific, behaviorally oriented treatment plan as required, with consumer involvement and signature. Monitoring confidentiality in accordance with Center policy and legal requirements. Completing required reports and evaluations. Following required office procedures in a cooperative manner. Initiating and processing incident reports as per Center guidelines. Providing case management and support to consumers. Treating consumers with respect. Providing support counseling, problem solving, contracts and limit setting. Establishing attainable goals with the consumer. Involving consumer, agencies and others in treatment planning. Collaborating with clinical team on engagement insight and needed supports. Providing support counseling with mental health issues, substance use disorders, and medical conditions. Assisting consumer in deciding which areas they want to address. Assisting and monitoring the group home consumers with medical interventions and hygiene needs. Encouraging consumers to attain the highest possible levels of independence. Accepting the values and lifestyles of consumers. Using a flexible approach with a diverse population. Working flexible hours, including evenings, weekends, holidays and during inclement weather. Making assessments and providing appropriate crisis interventions and risk management as needed to maintain a calm and safe environment. Assessing consumers' unmet needs. Identifying, assessing and responding to crisis situations. Teaching consumers adult daily living skills assisting them in all aspects of their life in the group home and supervised apartments. Teaching, supervising and assisting consumers with doing household chores using realistic situations. Teaching clients coping skills to implement symptoms management and harm reduction. Providing direct alcohol and drug counseling/case management to individuals with substance use disorders. Performing household duties as needed. Using a step-by-step process when teaching skills. Using positive reinforcement and encouragement. Integrating skills training in all aspects of their treatment plan. Maintaining a clean, healthy and safe environment at all times. Adhering to Infection Control protocols. Implementing emergency protocols when required. Transporting consumers in own vehicle to planned activities and appointments. Maintaining cooperative relationships with Center staff, consumers, community agencies and the public. Other duties as assigned.

Requirements include a high school diploma/GED required and/or at least 5 years of experience in a behavioral health setting. Experience in residential facilities is preferred. Must have a valid driver's license, registration, current inspection and insured automobile. Bilingual Spanish preferred.
