Job Summary

Responsible for providing nursing care to patients in their homes.

Essential Functions:

• Assessing patients' health status and developing individualized care plans.
• Administering medications and treatments as prescribed by the supervising registered nurse or physician.
• Monitoring patients' vital signs and reporting any changes to the supervising nurse or physician.
• Assisting patients with activities of daily living such as bathing, grooming, and dressing.
• Providing wound care and dressing changes.
• Performing basic diagnostic tests such as blood pressure checks and glucose monitoring.
• Educating patients and their families on their medical conditions and self-care techniques.

Qualifications

Education: Bachelor's Degree Related Field of Study required.

Can this role accept experience in lieu of a degree? No.

Licenses and Credentials:

• Class D Passenger Vehicle Driver's License MASS -required
• Licensed Practical Nurse [LPN - MASS -Required
• Completion of a state-approved practical nursing program and a current LPN license.

Knowledge, Skills and Abilities:

• Basic knowledge of medical terminology and procedures.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient demeanor.
• Reliable transportation to travel to patients' homes.
