Description Washington, DC | On-Site | $28.36 - $29.32 per hour | Washington Post Top Workplace (8x Winner) Community of Hope is seeking a Lead Patient Navigator to lead a team dedicated to removing barriers to care, connecting patients to vital resources, and helping them successfully navigate the healthcare system. What You'll Do Leads the Patient Navigation team while managing day-to-day effectiveness of the patient navigation service delivery model, which includes the evaluation of program outcomes and individual staff productivity and performance. Guides patients through various aspects of the healthcare system, ensuring they are well-informed and receive quality care with an emphasis on reducing barriers to health care access, providing tools for chronic disease self-management, and improving patients' quality of life. Identifies gaps in care and develops innovative and collaborative quality improvement initiatives to increase quality of care and enhance patient experience. Leads patient navigator in supporting and advocating for patients in attaining home health services by submitting home health requests to appropriate agencies on behalf of patients and providers. Follows up regularly to ensure timely processing of requests. Educates patients on medical transportation benefits and service utilization, schedules patient transportation for medical appointments as needed, and works with patients and medical providers to complete applications for Metro Access. Participates in team-based care and quality improvement initiatives as needed to promote population health and department goals. Meets with supervisees regularly and provides real-time feedback, training, support, and coaching as needed. Creates meeting agendas, sends meeting agendas in advance when possible, and documents concerns promptly as needed. Takes initiative and makes decisions within the scope of work. Assesses situations independently, thinks critically, and takes action to address issues or make useful recommendations while maintaining a solution-oriented lens. Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential. Requirements Must-Haves High school diploma, G.E.D, or equivalent experience required. BLS Certificate required. Excellent interpersonal skills, ability to show empathy, and ability to interact professionally and gain trust within a culturally diverse environment. Prior experience working with community resources and social services in D.C. Ability to approach patient support with a social determinants of health lens, understanding the impact of medical, social, and environmental disparities on patient health self-management; ability to support patient needs with empathetic cultural awareness. Excellent interpersonal skills, ability to show empathy, and ability to interact professionally and gain trust within a culturally diverse environment. Ability to handle confidential information with discretion and professionalism. Computer skills and proficiency in the use of electronic medical record and organizational communication software (Microsoft Suite). Ability to travel between COH sites or relocate to a different COH site on a full- or part-time basis. On-site: A majority or all of the position is required to work on-site. Proof of required vaccinations is required. This includes, but may not be limited to, Flu. At least two years of work experience in a clinical environment, social work, and/or community health; prior experience in public health or human services and/or community clinic or a similar role preferred. Bilingual (English/Spanish, English/French or English/Amharic) strongly preferred.
