RN Job Summary

As a Home Care RN, you will provide high-quality patient care in the field while also supporting clinical operations in the office. This role plays a key part in ensuring quality, compliance, and continuity of care.

Key Responsibilities

• Field Nursing Duties:
• Perform patient assessments, reassessments, and supervisory visits
• Provide skilled nursing care in clients' homes
• Implement and update individualized care plans
• Monitor and document changes in patient condition
• Educate clients and families on care needs
• Office / Clinical Support Duties:
• Conduct caregiver skills assessments and competency evaluations
• Assist with caregiver onboarding and orientation
• Review clinical documentation for accuracy and compliance
• Process and track physician orders
• Support coordination with scheduling and leadership team
• Communicate proactively with the Director of Nursing

Requirements

• Active, unrestricted RN license in Indiana
• CPR certification
• Negative TB test or chest X-ray
• Valid driver's license and auto insurance
• Ability to lift up to 50 lbs. and assist with mobility
• Home care or supervisory experience preferred but not required
• New graduates welcome with strong clinical skills

Pay and Schedule

Up to $45/hour (based on experience) Monday through Friday Occasional weekends or on-call support as needed
