Job Brief: Strong Force Staffing, a leading Recruitment Process Outsourcing (RPO) provider, is currently recruiting for the position of RN Case Manager for a valued client agency in Bethlehem. This vital field role entails direct patient care and case management within the community, aiming to optimize healthcare outcomes. This position offers competitive salary, on-call stipends, and mileage reimbursement. The final hiring decision will be made by our client agency.

Key Details:

• Salary: $85,000 - $95,000 Per Year, DOE
• Benefits: Mileage reimbursement
• Type: Field role

Responsibilities:

• Assess patient conditions and develop appropriate care plans.
• Maintain comprehensive medical records and report patient status to necessary parties.
• Act as the primary point of contact among patients, their families, and healthcare providers.
• Educate patients and families on health management and preventive measures.
• Coordinate with healthcare professionals to ensure continuous care and efficient case management.

Requirements:

• License: Active RN license in Pennsylvania with no restrictions
• Experience: At least 1 year in a similar in-home healthcare role.
• Skills: Proficient in case management software, excellent organizational and communication abilities.
• Traits: Empathetic, detail-oriented, and capable of autonomous operation in field settings.
